Community Care Access Centre (CCAC)
CCAC provides home care services for cancer patients. When home is no longer an option, they help people make the transition to other living arrangements. CCAC works in partnership with patients, families, providers and community organizations. Click here for a list of CCAC locations and contact information in the South West.
OHIP Billing Codes
Primary care practitioners can bill OHIP for a number of services involved in working with the CCAC, including referals, case conferences and completion of long-term care health reports. Click here for a listing of codes.
